Those are the Wisconsin single-trip oversize fees on the MV2600 chart for one movement. $15 is overlength only. $20 is overwidth or overheight. $25 is overwidth and overheight. Overweight uses the printed weight table and includes any oversize.

The same MV2600 puts two charges on top of the chart: “A charge of $10.00 per district will be made when, by reason of the size or weight of the vehicle or load, the route must be reviewed by a regional highway office.” “A charge of $10.00 per review will be made when, by reason of the gross weight of the vehicle and load, a review is necessary of specific highway structures.” We are not folding those into the chart totals.

“All charges for the transmission of an application or a permit, other than by U.S. Mail, are in addition to the permit fee and are the responsibility of the applicant.” Checkout, not the permit fee.

When this number took effect

MV2600 prints 6/2021 and “Permit fees are established by s.348.25 Wisconsin Statutes and Wisconsin Administrative Code Trans. 250.” We are not inventing a later date.
