Those are the typical Ohio one-way single-trip totals for one movement. $75 is $20 processing plus the $55 oversize surcharge (width up to 14' and/or height up to 14'6"). $145 is $20 processing plus the $125 overweight surcharge through 120,000 pounds.

OAC 5501:2-1-05: “(A) All permit application processing fees consist of the basic processing charge of twenty dollars plus each unit of surcharge that is applicable to that movement. (1) For one-way movements, the units of surcharge are as follows: (a) Overweight up to one hundred twenty thousand pounds … Outbound trip: one hundred twenty-five dollars … (c) Overall width up to fourteen feet and/or overall height up to fourteen feet six inches … Outbound trip: fifty-five dollars”

(B): “If a movement is both overweight and over width and/or over height, only one basic processing fee … and the applicable overweight surcharge … will be charged.”

Overweight over 120,000 pounds adds “one hundred twenty-five dollars plus four cents per ton over sixty tons, per mile traveled.” That per-mile term is adjacent, not the general headline.
